2004 Toyota Camry with approx 230k miles. Vibrates when accelerating 40 mph or above. Stops
vibrating when I let up off accelerator. Rides smooth when cruising. Only vibrates when accelerating.
Same issue 1 year ago. Replaced CV axles. Ran good approx. 7 months and started vibrating again. Any
ideas?

It sounds like a bad cv joint, that's for sure. First, check all 4 boots to make sure there is no damage. A small tear in a boot will allow dirt in, which wears out a cv joint very quickly indeed.
X2 on Michael's answer. All the other common causes of vibration would happen all the time.

the wheel bearings may be getting worn~ would check those for axial play~....how does it turn? kinda squishy?
