Can extreme cold freeze the heater fan, even though the truck started?

Oh ya, It just has to warm up to go like it should.
The only way the heater fan would "freeze up" is if the heater box was full of water. It's possible that your electrical connection is loose or you have a bad relay. The fan motor could also sieze up from age without water being the issue.

No, I live in the cold, when there is extreme cold the lubrication in the bearings gets thick and does not spin very well until it gets warmed up.

Otherwise the blower motor is bad, or needs a blower motor resistor.
