Wheel loosens on 2008 Toyota Tacoma

Three years ago, the right rear wheel of my 2wd Tacoma literally fell off while I was driving on a city street.  Mechanic who fixed it blamed the aluminum rim I had been using, said it flexes and loosens the bolts.  Since then, I have replaced the aluminum with steel.  Last week, the same wheel nearly fell off, this time it was the stock steel rim.  The truck is used lightly - has less than 30K miles. Lug nuts and studs have all been replaced since the original incident.  Wheels were mounted by a mechanic or by me, tightening lug nuts as much as possible with crappy Toyota lug wrench.  So - why is this happening?  What can be done to prevent it (short of Loctite)?

Good point. However, I'm using stock lug nuts on stock steel wheels, so have assumed they are correct.

Use a torque wrench and install the lug nuts dry with no lubrication or anti-seize. Also check the wheel and the vehicle mounting surfaces for corrosion and clean as necessary.
