mypower stering pump pulley has slide out and grinding on the car frame, what could cause this too happen?

my power steering pump pulley has slide out and grinding on the car frame, the pulley is on tight to the pump shaft. what could cause this too happen? the power steering pump belt is on the pulley.

The power steering pump shaft has come loose from the power steering pump. Pump needs to be replaced
