truck takes a long time to start below 50deg only. The colder the longer it takes

2 dealers have not been able to fix the problem. It also puffs black smoke on
start up and recently thrown a rich code on bank 2. In warm or hot weather it
doesn't do it. Dealer claims obstructed exhaust.

If the exhaust is obstructed why did the dealer not fix it? What does the dealer say about the rich code? That would explain the black smoke but not the hard starting.
