Why don't my gears work once my Savana has warmed up?

I have a 1997 Savana van. The gears stopped working all of a sudden and I thought the
trans. went out. I bought a rebuild transmission and installed it. Now when I turn on the
van it will shift gears but it seems like once the van has warmed up the gears will not shift
anymore. Once I let the van cool off and turn it back on the gears work again but once
again as soon as it warms up the van goes out again. What is the problem?
