What would cause my 2002 Windstar to blow anti-freeze out the back of the motor?

As I pulled in my driveway the was a large puff of smoke from under my hood. There was barely any fluid in my driveway and there is still coolant in the reservoir. There is anti-freeze blown on the firewall at the rear of the motor. The van was not overheating either. I was thinking that it may be a freeze plug but it's very warm out and I run 50/50 all the time. Help?!?!?!

I just replaced 2 freeze plugs on rear side of my 01 windstar. I put it on jack stands and installed rubber plugs that are tightened from bolt. Was a pain in the ass but I was able to get the hardest to replace, from what mechanics told me, freeze plug in
