I have a Nissan Titan SE 2005 , it has run good so far but when I start it now it is
sluggish to start and all the gauges rapidly fluctuate then the truck starts and runs
normal. Do I need A battery? Thanks for any help.

First check the battery terminal connections making sure they are clean and tight and do so general maintenance things check air filter and clean throttle body replace fuel filter add Lucas Fuel Injector Cleaner to gas tank.
Sounds like a classic case of a weak battery (probably one bad cell). If you can't test it yourself, take it to a local auto parts store and have them test it.
