Steer Assist Toyota RAV4

My 78 year old mother was driving on a highway on Saturday.  She said out of
nowhere the steering assist basically took over and forced her off the road as
she could not force the steering back.  The car rolled and she was upside down
but kicked a door out to get out of the car.  She said it was extremely scary but
it kind of came out of nowhere and happened so fast.  Is this a common issue?

No. The black box should have information on the vehicle's specifics at the time of the accident, your insurance company needs to be involved to get the information from a Toyota specialist in retrieving the information.
