What is causing door rattle?

2008 Sienna LE, 160,000 miles, well maintained, recently developed rattling noise in driver side front door when driving over bumpy roads.  Is there a usual cause for this problem that can be addressed by removing the door panel?  Thanks for your replies.

If nothing on the outside is moving around then pulling the panel may reveal something , It would be tough trying to locate the problem while driving over bumpy roads with the panel removed but that may be what you have to do .
